YOUR FIRST VISIT

On your initial visit, please plan to arrive at least 30 minutes before your scheduled appointment time to complete the necessary paperwork (Click here to see what forms you can fill out before you arrive).

PAYMENT AND INSURANCE

We will file all insurance claims for patients; however, the patient is responsible for payment of the deductible and copayment at the time of service. We are aware that illness is sometimes accompanied by financial difficulties and we will work with our patients who are experiencing these difficulties. Arrangements may be made after considerations of individual circumstances. However, you must make formal arrangements with our office before services are rendered. Once again, we will work with you regarding balances on your account, but ultimately, you are responsible for your bills as insurance coverage is a contract between you and your carrier.

Charges are based on the nature of your illness, and payment is due at the time of service.
